NEW STUDENT ENROLLMENT REQUIREMENTS
Please read carefully and follow the steps listed
A STUDENT WILL ONLY BE ENROLLED ONCE COPIES OF ALL REQUIRED
INFORMATION HAVE BEEN SUBMITTED TO THE GUIDANCE OFFICE AND VERIFIED
r STEP 1. NEW STUDENT INFORMATION -
Complete the “Census Data & Enrollment Questionnaire” form, and a “Home Language Survey”
form and leave it in the in-box for Mrs. Shobeiri, Guidance Specialist. Students enrolling from a foreign country must also provide official picture identification, indicating date of birth.
r STEP 2. RESIDENCE VERIFICATION –
Copies provided must reflect PARENT/GUARDIAN NAME & SERVICE ADDRESS.
Submit copies of a current gas or electric bill, or a receipt showing proof that the utility has been ordered. Phone and cable bills or bank statements, etc. are not acceptable.
No fixed, regular, adequate nighttime residence:
Homeless youth have the right to attend their school of origin or the school in the attendance area where he/she is temporarily living.
Sharing a residence:
If a student is living with the parent/guardian and residing in the home of another person a “SHARED RESIDENCE AFFIDAVIT” must be completed. This requires a meeting between the parent/guardian, an administrator and the person whose name appears on the residence verification. The parent/guardian and the owner/lessor/renter must present picture identification.
Living with a Guardian/Caregiver:
If a student is not living with the parent/legal guardian then a “CAREGIVER’S AUTHORIZATION AFFIDAVIT” must be completed by the receiving caregiver. The receiving caregiver must present picture identification. It is desired that the parent/guardian also present picture I.D. or a notarized statement releasing guardianship to the caregiver
r STEP 3. COMPLETE IMMUNIZATION RECORD – Submit immunization records to the school Health office.
Students who have not previously attended a "district feeder school" must also obtain a chest x-ray or TB mantoux test given within 60 days prior to enrollment.
Students must be cleared by the Health Office prior to enrollment.
r STEP 4. PRIOR SCHOOL ENROLLMENT -
Students must have a check-out slip from the last school attended indicating drop date, reason for withdrawal, and withdrawal grades. Students in grades 10-12 must also present a copy of an unofficial transcript. Freshmen must have a copy of their last report card.
